First Appointment

If this is your first visit to our clinic, we require a case history form to be filled out.

We have 3 options to complete this requirement:

​

1) You may download, print and fill-out this form ahead of your appointment and bring it with you to your appointment.

​

2) We have a Form Fillable PDF that you can fill out on the computer and email to us.

​

3) We also have forms in office.  If you choose this option, please arrive 5-10 mins before your appointment to fill out a case history form.

During your appointment Dr. Johnson will then be using this form and discussing with you your health history and the reason for your visit.  Following this he will preform an examination as well as appropriate adjustments.  X-rays are not required prior to treatment, however if you have access to fairly recent x-rays you are encouraged to bring them to your appointment.  Dr. Johnson's protocol is to adjust your problem areas a couple of times and if you are not significantly better after a few visits, then the need for x-rays will come into play for further diagnostic measures.
